Support at Home (SAH)

What is Support at Home?

The Support at Home (SAH) program, formerly the Home Care Packages Program and the Short-Term Restorative Care Programme, helps older Australians access coordinated services, equipment and home modifications to live safely and independently at home for longer.

Whether you need assistance with everyday tasks or more complex, ongoing care, Support at Home provides a personalised approach to help you maintain your independence and wellbeing.

Under Support at Home, you have access to a defined list of services:

Clinical Support

In-home nursing: Medication support, IV therapies, wound care, continence and tube feeding.

Allied health & therapeutic services: Personalised support to promote health, wellbeing and independence.

Nutrition: Support with prescribed nutrition and enteral feeding.

Personal care: Assistance with showering, dressing, daily activities, mobility and medications.

Care management: Coordinated support tailored to your individual needs.

Restorative care: Support to build confidence, regain abilities and maintain independence.

Everyday Living

Domestic assistance: Help with cleaning, laundry and shopping.

Home maintenance: Gardening, handyman services and minor repairs.

Meals: Fresh meal preparation and delivery.

Independence

Social connection: One-on-one support and community outings.

Assistive technology & home modifications: Equipment, installation, guidance, maintenance and repairs.

Transport: Support to get to appointments, activities and the community.

Respite: Giving carers time to recharge, knowing their loved ones are in capable, caring hands.

Therapeutic services: Access to services that support wellbeing and independence, including acupuncture, chiropractic, massage, art therapy and osteopathy.

Support at Home also includes services that support culturally safe care, such as help to join in cultural activities and access to Aboriginal and Torres Strait Islander health practitioners and health workers.

Support at Home Additional Short-Term Funded Services

We can also support you through three short-term Support at Home pathways, each designed to meet specific needs and provide additional funding where eligible. Two pathways can provide support for up to 12 weeks.

Restorative Care Pathway

Designed to help you build or regain skills and confidence, with support for up to 12 weeks, such as allied health services to promote independence and help you remain safely at home.

End-of-Life Pathway

Provides additional support for people nearing the end of life, helping them remain at home, surrounded by the people and things they love, with comfort, dignity and compassionate care, for up to 12 weeks.

Assistive Technology and Home Modifications (AT-HM) Scheme

Provides separate funding for equipment and home modifications that can make everyday life safer, easier and more comfortable. This may include mobility aids and other assistive equipment. HomeWise Health can help you access the scheme and coordinate the supply, installation and management of approved equipment and modifications.
